TERMS OF SERVICE

By completing this form, you are agreeing to the following terms of service that apply to all clients attending this practice:

  • Full payment of fees is expected on the day of appointment for all clients. Please note that even with a Mental Health Care Plan or other referral, you will still be out of pocket up to $140.00 per appointment. Please note that without a Mental Health Care Plan, private fees apply and you will be required to pay the full amount. 

  • All appointments that are rescheduled, cancelled or missed with less than 48 hours notice will incur a cancellation fee equal to the full session cost. Clients who exceed two such instances will be moved to the waiting list and may be required to make upfront payments for future rescheduling. 

  • All clients must agree to treat all members of the Windsor Allied Health team respectfully.

  • For any client who is NDIS funded, we will at a minimum require the plan dates and funding information before booking an appointment, as well as confirmation of who will pay the invoices and where they need to be sent. You are welcome to upload the plan as a file to this form.

  • Assessments will require a background session that is charged at the full rate of an appointment. This will in most cases not be eligible for any rebates.

Please let us know if there are any particular times that you are not available, any preferences around your practitioner, the location that you attend, or any other relevant information to assist us in allocating you to a practitioner. Please note that while we will endeavour to meet any requests, we can not guarantee that all will be met.

Your file has been allocated to Bev Ernst. Bev is a registered psychologist who works out of our Riverside office on Mondays and Tuesdays. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Cassie Lobley. Cassie is a provisional psychologist who works out of our South Launceston office Monday to Friday. Fees may differ depending on funding, however, the standard session fee for Cassie is $140.00 per session. As Cassie is a provisional psychologist, her sessions are not eligible for medicare rebates, however her fee is reduced to result in the same out-of-pocket amount. 

Your file has been allocated to Charl Fourie. Charl is a clinical psychologist who works out of our Riverside office, Monday, Tuesday and Thursday. Fees may differ depending on funding, however, the standard session fee is $240.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $137.05 for up to 10 sessions per calendar year. 

Your file has been allocated to Cherilee Botha. Cherilee is a counselling psychologist who works out of our South Launceston office Monday to Thursday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Gene Colgrave. Gene is a registered psychologist who works out of our South Launceston office on Saturday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Jolene Ridler. Jolene is a Clinical Social Worker who works out of our South Launceston office Thursday and Friday. Fees may differ depending on funding, however, the standard session fee is $120.00 per session. 

Your file has been allocated to Jozie De Deuge. Jozie is a registered psychologist who works out of our Riverside office on Mondays. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to JP Viviers. JP is a registered psychologist who works out of our South Launceston office Monday to Friday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Katie Lowe. Katie is a Behaviour Support Practitioner who works out of our Door of Hope office on Fridays, and also visits clients at home and in schools. Fees may differ depending on funding, however, the standard session fee is $193.99 per session.

Kerry is a registered psychologist who works out of our Riverside office, Tuesday to Friday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Lisa Van Zetten. Lisa is a Developmental Educator and Behaviour Support Therapist who works out of our Riverside office on Wednesday to Friday, and also visits clients at home and in schools. Fees may differ depending on funding, however, the standard session fee is $193.99 per session. 

Your file has been allocated to Mark van Helvoirt. Mark is a Speech Pathologist who works out of our Riverside office on Saturdays. Fees may differ depending on funding, however, the standard session fee is $120.00 per session. 

Your file has been allocated to Martine Plakalovic. Martine is a registered psychologist who works out of our Riverside office, Monday to Thursday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Nelria Yelland. Nelria is a clinical psychologist who works out of our Riverside office, Monday to Thursday. Fees may differ depending on funding, however, the standard session fee is $240.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $137.05 for up to 10 sessions per calendar year.

Rebecca is a registered psychologist who works out of our South Launceston office on Thursday and Friday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Sarah Barrett. Sarah is a registered psychologist who works out of our South Launceston office on Fridays and Saturdays of alternating weeks. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

Your file has been allocated to Tara Baillie. Tara is a Accredited Counsellor who works out of our South Launceston office on Monday and Tuesday. Fees may differ depending on funding, however, the standard session fee is $100.00 per session. 

Your file has been allocated to Tom Fegent. Tom is a registered psychologist who works out of our South Launceston office on Tuesday, Wednesday and Saturday. Fees may differ depending on funding, however, the standard session fee is $230.00 per session. With a mental health care plan from your GP, you are entitled to a rebate of $93.35 for up to 10 sessions per calendar year. 

If you agree to our Terms of Service, we will make a file in your name so that a first appointment can be booked. 

As a reminder, our terms are:

These Conditions of Service cover the matters of Fees and Broken Appointments.  Your signature below is taken as your acknowledgment and agreement to these Conditions of Service. Any variation must be made before your first appointment.

CONFIDENTIALITY

Windsor Allied Health collects personal information about you from you, your representative or a third party, using forms, online portals and other electronic or paper correspondence to collect this information. Windsor Allied Health may also be provided personal information by other organisations where referrals or collaboration are required. 

Windsor Allied Health, as service providers, will not ask you for any personal information which we do not need. The Privacy Act (1988) requires that we collect information for a purpose that is reasonably necessary for, or related to, a function or activity of Windsor Allied Health as a service provider.

All information and records regarding you are kept strictly confidential.  No information will be released to anyone without your authority.   If you were referred by a Medical Practitioner or other professional, I will usually send them an initial report and provide regular reports of your progress as this is normal professional courtesy.  It may be appropriate to liaise with them about related medical issues and medication.  If a child or adolescent was referred for assessment or intervention, reports and assessment results will be forwarded to the involved/collaborating Medical Practitioner (s), Specialist(s), Allied Health Worker(s), and School.  If you do not want this to happen or would like to have certain limitations imposed upon the amount of information conveyed, please discuss this with me at the first consultation.  Where the cost of your consultations is covered by an insurance company, regular reports are required by them.   You are welcome to a copy of any report that is written concerning you. If there are particular issues that come up in your therapy that you do not wish to have written about, please indicate this to me during your treatment.  If you are involved in a Court case and a psychologist appears on your behalf, or is subpoenaed to appear in Court, please note that psychologist’s records have no special legal privilege, I would not be allowed to withhold from the Court any information you have given me.  If you are involved in or are aware that you may have a possible legal issue arising related to the matter that you are seeking psychological help for, please inform me of this.  Psycho-legal assessments and consultations require a specific approach that can be different from a normal counselling or assessment session.

FEES

The current fee that is recommended by the Australian Psychological Society is $300.00 for a standard 50-minute session. My 50-minute sessions are charged at $230.00 per standard session. Fees for NDIS clients will be billed in line with NDIS guidelines. Shorter or longer appointments are charged proportionately less or more. Please contact Reception to enquire about fees for sessions less or more than 50 minutes. Payment is expected on the day of consultation. Payments may be made by cash, cheque, EFTPOS, Credit Card, or direct debit. If you have private health cover you may be able to claim a rebate for Psychological Services. If you are referred under a Mental Health Care Plan you will need to pay the full amount up front and you will be entitled to a rebate from Medicare for a maximum of 10 sessions per year. Any out-of-pocket expenses with be the responsibility of the client.

MEDICARE RULES

Medicare rebates can only be claimed for session when there is a current Mental Health Care Plan in place. Medicare requires that you return to your GP after the 6th visit under your mental health care plan for a re-referral from your GP for approval for more sessions. You will need to attend regular review appointments with your GP (usually after every 6 visits). In the case of children that have been referred to the practice under a Mental Health Care Plan, Medicare requires the child attends every session, there are no parent only sessions claimable under a Mental Health Care Plan. In the event that a parent requires a session without the child present, they will be billed privately & will be responsible for all out-of-pocket expenses that may occur.

CANCELLATION POLICY

Should you need to change or cancel an appointment it would be appreciated if you could allow 48 hours’ notice. All appointments that are rescheduled, cancelled or missed with less than 48 hours notice will incur a cancellation fee equal to the full session cost. The current fee is $100.00 for a standard one-on-one 50-minute session or $150.00 for a couples counselling session. Payment is expected on the day of consultation. Fees for NDIS clients will be billed in line with NDIS guidelines. Currently there is no Medicare or Private Health rebate claimable for my sessions, but we do keep our fees low and therefore, you will only be charged the out-of-pocket expense you would receive if using a MHCP or Health Insurance, which is $100.00 for a 50-minute session.

Shorter or longer appointments are charged proportionately less or more. Please contact Reception on 6327 5701 to enquire about fees for sessions shorter or longer than 50 minutes. Payments may be made by cash, cheque, EFTPOS, or Credit Card. Any out-of-pocket expenses will be the responsibility of the client.

Fees for NDIS clients will be billed in line with NDIS guidelines. Shorter or longer appointments are charged proportionately less or more. Please contact Reception to enquire about fees for sessions less or more than 50 minutes. Payment is expected on the day of consultation. Payments may be made by cash, cheque, EFTPOS, Credit Card, or direct debit. If you have private health cover you may be able to claim a rebate for Psychological Services. If you are referred under a Mental Health Care Plan you will need to pay the full amount up front and you will be entitled to a rebate from Medicare for a maximum of 10 sessions per year. Any out-of-pocket expenses with be the responsibility of the client.

Medicare requires that you return to your GP after the 6th visit under your mental health care plan for a re-referral from your GP for approval for more sessions. In the case of children that have been referred to the practice under a Mental Health Care Plan, Medicare requires the child attends every session, there are no parent only sessions claimable under a Mental Health Care Plan. PROVISIONAL PSYCHOLOGIST

A Provisional Psychologist is someone who has completed their tertiary qualifications and is undertaking a program of supervision with a senior psychologist as they develop their applied skills. This means you or your child’s case notes will be discussed with Charl Fourie on a weekly basis to provide the provisional psychologist with guidance and best practice strategies. FEES

The current fee that is recommended by the Australian Psychological Society is $300.00 for a standard 50-minute session. Windsor's fee for registered Psychologists is $230, which usually includes a rebate from Medicare for those with referrals from GPs. Due to Cassie's provisional registration, you will only be charged the out-of-pocket expense, which is $140.00 for a 50-minute session. Payment is expected on the day of consultation.  Fees for NDIS clients will be billed in line with NDIS guidelines. 

Shorter or longer appointments are charged proportionately less or more. Please contact Reception to enquire about fees for sessions less or more than 50 minutes. Payments may be made by cash, cheque, EFTPOS, or Credit Card. Any out-of-pocket expenses with be the responsibility of the client.

CANCELLATION POLICY

Should you need to change or cancel and appointment it would be appreciated if you could allow 48 hours’ notice. If you cancel within 48 hours,you will be charged 50% of your appointment fee. If you cancel within 24 hours of your scheduled appointment or do not attend your scheduled appointment,you will be charged the full appointment fee. If you need to cancel for emergencies or sudden illness, the cancellation fee may be waived. Cancellations can be made by contacting Reception on 6327 5701. If there is no answer, please leave a message and your call will be returned as soon as possible.

ThePrivacy Act (1988) requires that we collect information for a purpose that is reasonably necessary for, or related to, a function or activity of Windsor Allied Health as a service provider.

All information and records regarding you are kept strictly confidential. No information will be released to anyone without your authority. If you were referred by a Medical Practitioner or other professional, I will usually send them an initial report and provide regular reports of your progress as this is normal professional courtesy. It may be appropriate to liaise with them about related medical issues and medication. If a child or adolescent was referred for assessment or intervention, reports and assessment results will be forwarded to the involved/collaborating Medical Partitioner(s), Specialist(s), Allied Health Worker(s) and School. If you do not want this to happen or would like to have certain limitations imposed upon the amount of information conveyed, please discuss this with me at the first consultation. Where the cost of your consultations is covered by an insurance company, regular reports are required by them. You are welcome to a copy of any report that is written concerning you. If there are particular issues that come up in your therapy that you do not wish to have written about, please indicate this to me during your treatment. If you are referred under a Mental Health Care Plan you will need to pay the full amount up front and you will be entitled to a rebate from Medicare for a maximum of 10 sessions per year. Any out-of-pocket expenses with be the responsibility of the client.

MEDICARE RULES

Medicare rebates can only be claimed for session when there is a current Mental Health Care Plan in place. Medicare requires that you return to your GP after the 6th visit under your mental health care plan for a re-referral from your GP for approval for more sessions. You will need to attend regular review appointments with your GP (usually after every 6 visits). In the case of children that have been referred to the practice under a Mental Health Care Plan, Medicare requires the child attends every session, there are no parent only sessions claimable under a Mental Health Care Plan. In the event that a parent requires a session without the child present, they will be billed privately & will be responsible for all out-of-pocket expenses that may occur.

A provisional psychologist is someone who has completed their tertiary qualifications and is undertaking a program of supervision with a senior psychologist as they develop their applied skills. This means you or your child’s case notes will be discussed with a supervisor on a weekly basis to provide the provisional psychologist with guidance and best practice strategies.
